A terapia ventilat???ria n???o-invasiva ??? frequentemente oferecida para ajudar os idosos com insufici???ncia respirat???ria aguda (IRA) no departamento de emerg???ncia. O nosso objectivo era real???ar as dificuldades encontradas durante esta pr???tica e os factores preditivos do fracasso. O estudo foi retrospectivo, realizado no departamento de emerg???ncia do CHU La Rabta (Tun???sia). Setenta e cinco pacientes com mais de 65 anos de idade com ARF e recurso ??? ventila??????o n???o-invasiva (VNI) foram inclu???dos. Os diagn???sticos etiol???gicos da IRA foram frequentemente associados a 72% de descompensa??????o aguda da DPOC, 61% de insufici???ncia card???aca aguda, 37% de pneumopatia e 1% de embolia pulmonar. A falha da VNI resultou em 24% de mortalidade hospitalar e 8% de admiss???o na unidade de cuidados intensivos. Hipertens???o anterior (p=0,026), refluxo hepato-jugular (p=0,028), edema dos membros inferiores (p=0,016), uraemia (p=0,005), CRP (p=0,004) e pH inicial (p=0,044) foram correlacionados com a mortalidade. Assim, foram identificados no nosso estudo v???rios factores preditivos para o fracasso da terapia ventilat???ria n???o-invasiva. A frequ???ncia das comorbilidades agudas poderia explicar as dificuldades encontradas.
