Die Entdeckung oder Entwicklung fast aller bisherigen Antiepilep- tika beruht auf mehr oder weniger zufiilligen Beobachtungen bei Patienten (wie bei Phenobarbital) oder im Tierexperiment (wie bei Carbamazepin oder Valproinsiiure) bzw. auf von diesen Beobachtun- gen ausgehenden pharmakologischen Weiterentwicklungen (wie bei Phenytoin, Ethosuximid oder Primidon). Der genaue molekulare Wirkmechanismus dieser Substanzen ist trotz zahlreicher Untersu- chungen nach wie vor nicht bekannt, und nach dem derzeitigen Wis- sensstand ist anzunehmen, daB sie mehr als einen Angriffspunkt ha- ben. Demgegeniiber ist Vigabatrin das erste Antiepileptikum, bei dem die Vorstellung iiber den Wirkmechanismus am Anfang und der Nachweis der klinischen Wirksamkeit am Ende einer gezielten Ent- wicklung stand. Nach rund 2 lahrzehnten ohne EinfUhrung neuer Substanzen wur- de Vigabatrin in Deutschland Ende 1991 vom Bundesgesundheitsamt zugelassen und steht seit Anfang 1992 zur Zusatzbehandlung von bislang nur unzureichend beeinfluBbaren Epilepsien zur Verfiigung.
