When the Keaton Batman movies came out for some reason the Joker's real name was Jack, the only time the Joker was referred to as anything but Joker. Because of that movie, and probably Jack Nicholson, no one was ever supposed to refer to the Joker as Jack, until McCammon's story accidently slipped into print.
